Assessment

Our goal focused risk assessments and unique assessment tools deliver measurable plans to address the complex physical and psychological needs of our clients.

After an initial assessment of the client’s individual needs, we produce a rehabilitation plan for a case manager to co-ordinate in partnership with the client, family and professionals involved.

Our initial assessments and reports get the balance right. They are informed and insightful, whilst being concise and to the point. It is important at this stage to convey a high level of accurate information to mitigate any future risk.

Our recommendations clearly define problem areas, solutions and costs, including case management objectives and an estimate of the hours required. Regular reporting is essential and can be provided on a monthly or quarterly basis as preferred. Referrers are always kept up to date on any activity that is likely to exceed the original estimate.

We amend and review plans continuously to take account of any changes that might arise and are dedicated to ensuring that the service providers deliver programmes as planned. If changes are required, we ensure that these are fully reasoned and implemented.

Case Managers

Our team of case managers realise the importance of having the time available to carry out the role properly and often go that extra mile to achieve the desired results. They manage and co-ordinate the delivery of plans efficiently and effectively with all the parties involved.

We pride ourselves on being able to match the right case manager with the client. This allows for a positive relationship to develop enabling the smoothest rehabilitation and transition to as normal a life as is possible. The proactive nature of our team allows for a flexible ability to reset client plans if the current regimen isn’t working. Coupled with continually measuring achievement through rigorous goal setting we can manage your risk expectations while enabling us to assist clients in getting the best possible and speediest solution for their needs.

We provide comprehensive signposting to the full range of proven and trusted service providers. We manage the full spectrum of people involved with the case and often work with people that we have known and worked with over many years, having built trusted relationships.

We know that it is a critical part of our job to understand clearly what third parties provide, how effectively they deliver and that we are able to clearly monitor outcomes.
