Immediate Needs Assessment

Our Immediate Needs Assessment provides all parties with a clear picture of the input required to maximise functional recovery. We include a full rationale and estimated costs for all our recommendations.

  • Care

    Review of personal care, day to day support in the home and accessing the local community, attending therapy appointments.

  • Medical

    Review of medical interventions in place and whether clients' needs are being met. Unmet needs are explored and recommendations made including specialist services.

  • Rehabilitation

    Review of past/existing rehabilitation and additional support may be beneficial, for example physio and occupational therapy, music and play therapy for children.

  • Equipment

    Assessing, exploring and sourcing equipment as the clients’ needs change.

  • Accommodation

    A review of the home environment will establish whether clients are able to continue to access their property, it may be that adaptations are required or more suitable accommodation is explored with the client.

  • Vocation, Employment & Leisure

    Exploring whether clients can attend work or education and how they may enjoy holidays, hobbies or other leisure activities.

  • Transport

    Assessing availability, suitability and accessibility of transport options for the client, their carers and family.

  • Case Management

    The case manager will work closely with the client and their family in carrying out the recommendations from the Immediate Needs Assessment report.
